Reason for Use code 398 For short-term use in women who are experiencing symptoms of menopause. Note: Recent evidence has demonstrated that use of hormone replacement therapy (HRT) increases the rate of coronary events, breast cancer, dementia, stroke, venous thromboembolism and referrals for abnormal vaginal bleeding. These risks should be discussed with patients and reviewed periodically. Coverage limit: LU Authorization Period: 1 year

INDICATIONS AND CLINICAL USE Premarin® Vaginal Cream is indicated in the treatment of atrophic vaginitis, dyspareunia, and kraurosis vulvae. PREMARIN® VAGINAL CREAM HAS NOT BEEN SHOWN TO BE EFFECTIVE FOR ANY PURPOSE DURING PREGNANCY AND ITS USE MAY CAUSE SEVERE HARM TO THE FETUS. Premarin® Vaginal Cream should be prescribed with an appropriate dose of a progestin for women with intact uteri in order to prevent endometrial hyperplasia/carcinoma Geriatrics (> 65 years of age): See above indications.
