Coverage limit: For the treatment of patients with: Reason for Use code 362 Methicillin-resistant Staphylococcus species (MRSA, MRSE) infections* in patients who are intolerant or have failed vancomycin therapy, or have contraindications to venous access. Coverage limit: LU Authorization Period: 1 year Reason for Use code 363 Vancomycin resistant Enterococcus species (VRE) infections* in patients switching from IV linezolid. Coverage limit: LU Authorization Period: 1 year Reason for Use code 364 Step-down therapy for the treatment of methicillin-resistant Staphylococcus species or vancomycin resistant Enterococcus species (VRE) infections* after parenteral therapy or hospital/ emergency department discharge. Coverage limit: * Infections must be documented and culture proven. Not approved for colonization (e.g. nares, urine, etc). Maximum 28 days supply. Coverage limit: LU Authorization Period: 1 year

1 INDICATIONS ZYVOXAM (linezolid injection and powder for oral suspension) is indicated for: Treatment of adult patients with the following infections, when caused by susceptible strains of the designated aerobic Gram-positive micro-organisms: Note: ZYVOXAM is not indicated for the treatment of Gram-negative infections. It is critical that specific Gram-negative therapy be initiated immediately if a concomitant Gram-negative pathogen is documented or suspected (see 7 WARNINGS AND PRECAUTIONS). Vancomycin-Resistant Enterococcus faecium (VREF) Infections: ZYVOXAM is indicated for the treatment of the following infections when due to VREF: • Intra-abdominal, skin and skin-structure, and urinary tract infections (including cases associated with concurrent bacteremia) (see 14 CLINICAL TRIALS). Note: This indication for VREF is based on non-comparative studies.
