Reason for Use code 371 For the prophylaxis (max: 75mg daily) of institutionalized individuals during confirmed* outbreaks of Influenza A or Influenza B. Coverage limit: Network will limit supply to 6 weeks. Coverage limit: *The outbreak must be confirmed by Public Health. Coverage limit: LU Authorization Period: 1 year Reason for Use code 372 For the treatment (max: 75mg bid) of institutionalized individuals during confirmed* outbreaks due to: Influenza B or, Influenza A (as an alternative to amantadine) or, Influenza A where new cases have developed despite amantadine prophylaxis. Coverage limit: Network will limit supply to 5 days. Coverage limit: *The outbreak must be confirmed by Public Health. Coverage limit: LU Authorization Period: 1 year Reason for Use code 639 For treatment of individuals who are at high risk* of complications from influenza infection and have either: 1. Laboratory-confirmed influenza A or influenza B infection; OR 2. Illness consistent with influenza A or influenza B infection *High risk of complications from influenza infection is defined by the presence of one or more of the following medical conditions, age-related factors, or other characteristics: - Asthma and other chronic pulmonary diseases, including bronchopulmonary dysplasia, cystic fibrosis, chronic bronchitis, and emphysema - Cardiovascular disease (excluding isolated hypertension), including congenital and acquired heart disease, such as heart failure and symptomatic coronary artery disease - Renal disease - Chronic liver disease - Diabetes mellitus and other metabolic diseases - Anemia and hemoglobinopathies, such as sickle cell disease - Cancer, immunosuppression, or immunodeficiency due to disease (e.g., HIV infection, especially if CD4 is less than 200 per microlitre) or management of underlying conditions (e.g., solid organ transplant or hematopoietic stem cell transplant recipients, those receiving immunosuppressive therapies for autoimmune conditions or other disorders) - Neurological disease and neurodevelopmental disorders that compromise handling of respiratory secretions (cognitive dysfunction; spinal cord injury; neuromuscular, neurovascular, neurodegenerative, and seizure disorders; cerebral palsy; metabolic disorders) - Children aged younger than 5 years - Individuals aged 65 years or older - Individuals of any age who are residents of nursing homes or other chronic care facilities - Pregnancy and up to 4 weeks postpartum regardless of how the pregnancy ended - Obesity with a body mass index (BMI) greater than or equal to 40 or a BMI greater than 3 z-scores above the mean for age and gender - Children and adolescents aged younger than 18 years undergoing treatment for long periods with acetylsalicylic acid - Indigenous peoples Maximum dosage: 75mg twice daily for 5 days Treatment should be initiated as soon as possible (ideally no more than 48 hours) after onset of symptoms to achieve optimal benefits. Lowest DBP recovery rule under medically necessary no-substitution claims: 6.2 Medically Necessary "No Substitution" Claims The Side Effect Reporting Form will not have to be renewed. However, the pharmacy must maintain a copy of the prescription that contains a direction that there be no substitution and the required Health Canada Side Effect Reporting Form (completed and signed by the prescriber). The prescriber must write “No Substitution” or “No Sub” on renewal or subsequent new written prescriptions, and indicate “No Substitution” on subsequent new oral prescriptions. The dispenser will be reimbursed the DBP plus a mark-up and the lesser of the posted usual and customary fee or the ODB dispensing fee minus the applicable ODB co-payment amount. Where a completed Side Effect Reporting Form is not available at the pharmacy during an inspection, the difference between the cost of the higher-cost product and the lowest DBP listed for the interchangeable category will be recovered. (ontario.ca, Revision #17; 2026-08-03 ↗) 6.2 Medically Necessary "No Substitution" Claims The Ministry will provide reimbursement of a higher-cost interchangeable product in medically necessary circumstances where a patient has experienced a significant adverse reaction with two (2) lower-cost interchangeable drug products, where available. When a prescriber identifies a patient for which it is medically necessary that a higher cost interchangeable product be provided, the prescriber must: • Complete, sign and forward to the pharmacist a copy of the Health Canada side effect reporting form for each lower-cost interchangeable drug product trialed (Side Effect Reporting Form[s]); and • Write “No Substitution” or “No Sub” on a written prescription or indicate “No Substitution” to the pharmacist in the case of a verbal prescription. The prescriber should keep a copy of the completed form in the patient’s record for future use and reference. In the case of a written prescription, when the pharmacist or dispensing physician receives a prescription with the written notation “No Substitution” or “No Sub”, reimbursement will be provided for the higher-cost interchangeable product only if the prescription is accompanied by a completed Health Canada Side Effect Reporting Form for each of the lower-cost interchangeable drug products trialed. This form must be completely filled out noting the details of the adverse reaction and signed by the prescriber. In the case of a verbal prescription, the prescriber must satisfy the operator of the pharmacy or dispensing physician that a completed Health Canada Side Effect Reporting Form for each of the lower-cost interchangeable drug products trialed has been completed and signed by the prescriber. A written record of this verbal prescription and the completed Health Canada Side Effect Reporting Form must be received by the pharmacy prior to claim submission. Upon receipt, the pharmacist must: • Fax, submit online or mail the completed and signed form to Health Canada’s Canada Vigilance Program; and • Retain his or her copy of the completed and signed Side Effect Reporting Form. 1. Indications TAMIFLU (oseltamivir phosphate, capsules and powder for oral suspension) is indicated for: • The treatment of uncomplicated acute illness due to influenza infection in adults and adolescents (≥ 13 years) who have been symptomatic for no more than 2 days. The treatment indication is based on two Phase III clinical studies of naturally occurring influenza in adults in which the predominant infection was influenza A (95%) and a limited number with influenza B (3%) and influenza of unknown type (2%), reflecting the distribution of these strains in the community. The indication is also supported by influenza A and B challenge studies. No data are available to support the safety and efficacy of TAMIFLU in adult patients who commenced treatment after 40 hours of onset of symptoms. • The treatment of uncomplicated acute illness due to influenza in pediatric patients 1 year and older who have been symptomatic for no more than 2 days.
