Reason for Use code 578
For the management of opioid use disorder in combination with counseling and psychosocial support in adult patients who meet the following criteria:
- The patient is stabilized on a dose of no more than 8mg per day of sublingual buprenorphine for the preceding 90 days; AND
- The patient is under the care of a health care provider with experience in the diagnosis and management of opioid use disorder and has been trained to implant and remove the buprenorphine subdermal implant.
Recommended dose: Four 80mg implants inserted subdermally in the inner side of the upper arm for up to six months.
The maximum quantity that can be claimed per patient is four (4) implant cycles (i.e. two (2) years of therapy).
NOTE: The product monograph indicates that dosing beyond two (2) years cannot be recommended at this time. Probuphine subdermal implants are intended to be in place for six months of treatment. Probuphine implants are removed at the end of the six-month period. If continued treatment is desired, the implants should be replaced by new implants (implanted in the opposite arm) at the time of removal.

adverse reaction with two (2) lower-cost interchangeable drug products, where
available. When a prescriber identifies a patient for which it is medically necessary
that a higher cost interchangeable product be provided, the prescriber must:
• Complete, sign and forward to the pharmacist a copy of the Health Canada
side effect reporting form for each lower-cost interchangeable drug product
trialed (Side Effect Reporting Form[s]); and
• Write “No Substitution” or “No Sub” on a written prescription or indicate “No
Substitution” to the pharmacist in the case of a verbal prescription.
The prescriber should keep a copy of the completed form in the patient’s record for
future use and reference.
In the case of a written prescription, when the pharmacist or dispensing physician
receives a prescription with the written notation “No Substitution” or “No Sub”,
reimbursement will be provided for the higher-cost interchangeable product only if
the prescription is accompanied by a completed Health Canada Side Effect
Reporting Form for each of the lower-cost interchangeable drug products trialed.
This form must be completely filled out noting the details of the adverse reaction
and signed by the prescriber.
In the case of a verbal prescription, the prescriber must satisfy the operator of the
pharmacy or dispensing physician that a completed Health Canada Side Effect
Reporting Form for each of the lower-cost interchangeable drug products trialed
has been completed and signed by the prescriber. A written record of this verbal

6.2 Medically Necessary "No Substitution" Claims
The Ministry will provide reimbursement of a higher-cost interchangeable product in
medically necessary circumstances where a patient has experienced a significant
adverse reaction with two (2) lower-cost interchangeable drug products, where
available. When a prescriber identifies a patient for which it is medically necessary
that a higher cost interchangeable product be provided, the prescriber must:
• Complete, sign and forward to the pharmacist a copy of the Health Canada
side effect reporting form for each lower-cost interchangeable drug product
trialed (Side Effect Reporting Form[s]); and
• Write “No Substitution” or “No Sub” on a written prescription or indicate “No
Substitution” to the pharmacist in the case of a verbal prescription.
The prescriber should keep a copy of the completed form in the patient’s record for
future use and reference.
In the case of a written prescription, when the pharmacist or dispensing physician
receives a prescription with the written notation “No Substitution” or “No Sub”,
reimbursement will be provided for the higher-cost interchangeable product only if
the prescription is accompanied by a completed Health Canada Side Effect
Reporting Form for each of the lower-cost interchangeable drug products trialed.

1 INDICATIONS
SUBLOCADE (buprenorphine solution (extended-release) for injection) is indicated for the
management of moderate to severe opioid use disorder in adult patients.
SUBLOCADE should be used as part of a complete treatment plan that includes counselling
and psychosocial support.
SUBLOCADE must only be administered subcutaneously by a healthcare professional (see 4
DOSAGE AND ADMINISTRATION).
1.1 Pediatrics
Pediatrics (˂18 years of age): No data are available in patients under 18 years of age.
SUBLOCADE is not indicated in pediatric patients.
1.2 Geriatrics
Geriatrics (≥ 65 years of age): There were no patients ≥ 65 years of age in the controlled
clinical trial of SUBLOCADE. In general, drug use for an elderly patient should be cautious,
reflecting the greater frequency of decreased hepatic, renal, respiratory and/or cardiac function,
concomitant disease or other drug therapies. If the decision is made to prescribe SUBLOCADE
to individuals 65 years of age or older, patients should be monitored for signs and symptoms of
toxicity or overdose.
