1 INDICATIONS MINT-ITRACONAZOLE (itraconazole) capsules are indicated for: • the treatment of the following systemic fungal infections in normal, predisposed or immunocompromised patients: 1. Invasive and non-invasive pulmonary aspergillosis. 2. Oral and/or esophageal candidiasis. 3. Chronic pulmonary histoplasmosis. 4. Cutaneous and lymphatic sporotrichosis. 5. Paracoccidioidomycosis. 6. Chromoblastomycosis.(formerly chromomycosis) 7. Blastomycosis. The type of organism responsible for the infection should be isolated and identified and other relevant laboratory studies (wet mount, histopathology, serology) should be undertaken as appropriate to confirm diagnosis. Note: Requests for cancer drugs are considered under the Case-by-Case Review Program (CBCRP) which is administered by Ontario Health (Cancer Care Ontario) on behalf of the Ministry of Health. The CBCRP considers funding requests for drugs (both oral therapies and injectable drugs) for the treatment of cancer in patients who have a rare clinical circumstance that is immediately life-threatening (death is likely within a matter of months) and who require treatment with an unfunded drug, because there is no other satisfactory and funded treatment.
