Reason for Use code 384 For use in combination with an alpha blocker for the treatment of men with symptomatic* Benign Prostatic Hyperplasia. Coverage limit: LU Authorization Period: Indefinite Reason for Use code 385 For monotherapy, as a second line agent in patients with symptomatic* Benign Prostatic Hyperplasia following treatment failure or intolerance to an alpha blocker. Coverage limit: * Symptomatic is defined as having moderate (about half the time) to severe (almost always) symptoms related to the prostate in at least 4 of the following domains: 1. feeling of incomplete emptying of the bladder after voiding 2. needing to urinate again less than 2 hours after previous void 3. stopping and starting urine several times while voiding 4. difficulty postponing urination 5. weak urinary stream 6. pushing or straining to begin voiding 7. the need to get up to void at least 3 times in the night. What the Health Canada label says it is for
1. Indications Monotherapy: AVODART (dutasteride) is indicated for the treatment of symptomatic Benign Prostatic Hyperplasia (BPH) in men with enlarged prostates. Combination therapy: AVODART (dutasteride) in combination with the alpha blocker, tamsulosin, is indicated for the treatment of moderate to severe symptomatic BPH in men with enlarged prostates. AVODART administered alone or in combination with the alpha blocker, tamsulosin, has been shown to reduce prostate size, improve urinary flow and symptoms of BPH. AVODART administered as monotherapy has been shown to reduce the risk of acute urinary retention (AUR) and the need for BPH related surgery. Combination therapy was statistically significant to tamsulosin monotherapy but not dutasteride monotherapy at reducing the relative risk of AUR or BPH- related surgery (see 14 Clinical Trials). Limitations of Use: AVODART is not approved for the prevention of prostate cancer. 1.1. Pediatrics BPH is not a disease of childhood. AVODART is contraindicated for use in children.
