Reason for Use code 399 Outpatient continuation of treatment for documented invasive aspergillosis in patients who have demonstrated a clinical response to either oral or parenteral voriconazole. * The first prescription must be written by a physician based at the hospital where the patient was hospitalized. Note: Limited to 3 months of reimbursement. 1 INDICATIONS VFEND (voriconazole) is indicated for: • Invasive aspergillosis • Candidemia in non-neutropenic patients and the following Candida infections: disseminated infections in skin and infections in abdomen, kidney, bladder wall and wounds (see 14 CLINICAL TRIALS and 15 MICROBIOLOGY). 1.1 Pediatrics Pediatrics (<12 years of age). Based on the data submitted and reviewed by Health Canada, the safety and efficacy of VFEND in pediatric patients below 12 years of age have not been established; therefore, Health Canada has not authorized an indication for use in these patients.
