Ontario Limited Use criteria — code 472
Reason for Use code 472 For the management of cystic fibrosis patients with chronic pulmonary Pseudomonas aeruginosa (P. aeruginosa) infections. Tobramycin is administered in alternating periods of 28 days. After 28 days of therapy, patients should stop therapy for the next 28 days, and then resume therapy for the next 28 day on / 28 day off cycle. Coverage limit: LU Authorization Period: Indefinite

INDICATIONS AND CLINICAL USE TOBREX (Tobramycin Ophthalmic Solution, USP) and TOBREX (Tobramycin Ophthalmic Ointment, USP) are topical antibiotic preparations indicated in the treatment of external infections of the eye and its adnexa caused by susceptible bacteria. Appropriate monitoring of bacterial response to topical antibiotic therapy should accompany the use of TOBREX.
