Reason for Use code 475 For reducing the risk of overt hepatic encephalopathy (HE) recurrence (i.e., 2 or more episodes) in patients who are unable to achieve adequate control of HE recurrence with maximal tolerated dose of lactulose alone. Rifaximin should be used in combination with a maximal tolerated dose of lactulose. For patients not maintained on lactulose, the nature of the patient's intolerance to lactulose should be documented. Coverage limit: LU Authorization Period: Indefinite

What the Health Canada label says it is for
1 INDICATIONS ZAXINE is indicated for: • the treatment of irritable bowel syndrome with diarrhea (IBS-D) in adults. • the reduction in risk of overt hepatic encephalopathy (HE) recurrence in patients ≥ 18 years of age. In the trials of ZAXINE for HE, 91% of the patients were treated with lactulose concomitantly or were on lactulose treatment concomitantly. Differences in the treatment effect on those patients not using lactulose concomitantly could not be assessed. ZAXINE has not been studied in patients with MELD (Model for End-Stage Liver Disease) scores > 25, and only 8.6% of patients in the controlled trial had MELD scores over 19. There is increased systemic exposure to rifaximin in patients with hepatic dysfunction.
